Conflict Disclosure Forms
Chapter 176, Texas Local Government Code, requires local government officers to file a conflicts disclosure statement under certain circumstances. A council member or other City government official must file a questionnaire (Form CIS) with the City if the officer or the officer's family member has an employment or business relationship with a business that has contracted with the City or the City is considering doing business with and the employment or business relationship has resulted in taxable income that exceeds $2,500 during the preceding 12 months.
